HealthFirst Medical Group is pleased to have several
Physician Assistants
(PA's) and Nurse Practitioners (NP's) as part of our health care team.
PA's are highly skilled professionals who provide medical services that would
otherwise be performed by a doctor. Our PA's work under the supervision of
our physicians and work diligently to ensure high quality care. They are board
certified by the NCCPA and licensed by the TSBME.
Nurse Practitioners are trained to promote health by teaching people to stay
healthy, as well as treating them for acute illnesses and chronic
diseases. They work in collaboration with our physicians to ensure you
receive high quality health care. They are certified and licensed by the BNE.
Our Clinics often offer health care by Physician
Assistants and Nurse Practitioners. If at any time
you wish to see a Medical Doctor, please notify the front desk and we will be
happy to assist you with this request.

We encourage our patients to schedule appointments in advance whenever
possible. We agree that your time is valuable and work very hard to see
our patients on time. However, please understand that delays and
emergencies occur. If you are unable to keep a scheduled appointment,
please provide our office with 24 hours advance notice. We realize with
today's busy schedules you may need to come in without an appointment. We welcome you to do
so at our
Family Practice Clinics. Patients with an appointment will be seen first, and those without an
appointment will be seen as soon as possible. Emergencies will receive immediate
triage assessment.

We consider your relationship with HealthFirst Medical Group to be a
confidential matter. Authorization forms are available should you wish any
information to be released to a third party.

Prescriptions by telephone without an office visit are kept to a minimum for
your safety. We prefer to examine you prior to prescribing medication to
ensure both your safety and speedy recovery from illnesses. Should you need a
refill on a prescribed medication, please call your pharmacy at least three
business days in advance. The pharmacy will contact our office if
authorization is required. Routine medication refills take up to 72
hours. Please make sure to request refills well in advance, as they will
not be handled after hours.

In the event of a medical problem after hours, please call the clinic where
you are a patient. Our answering service will obtain brief information and
contact the physician or Physician Assistant on call to address your need. If emergency care is
needed, go directly to the most appropriate hospital emergency room.
Please remember to remove any call blocking features on your phone so that we
can return your call promptly.

Charges for office visits vary according to the complexity and severity of
the problem being addressed. Payment for office services rendered is
expected at the time of your visit. Cash, check and credit cards are
accepted.

If you are admitted to a downtown Ft. Worth hospital, your care will be
provided by Hospital Specialist Physicians. The Hospitalist will coordinate your inpatient care to include
admission, daily rounds, discharge and follow-up care. The Hospitalist
will communicate with your Primary Care physician as needed and forward all
information regarding your care to him or her.

It is important that your records be kept current and accurate. We ask
each patient to complete an updated information sheet every year. Please
notify our office of any changes in your health status, address, telephone
number or health insurance.
